Referral Process
1. Complete the Referral Form
Use our online referral form --OR-- download a copy and complete.
2. Submit Referral Information
Submit the referral form along with all required documentation from the Referral Checklist and submit online or via:

3. Next Steps
Our team will verify insurance after receiving all documentation. Once verified, the patient will be scheduled for an initial visit within 2–3 business days.
4. Ongoing Communication
Weekly progress reports will be sent to the referring provider.
