General Information

Advanced mobile wound care is a specialized medical service that brings expert wound care directly to patients in their homes, assisted living facilities, or long-term care settings. Our clinic is equipped with the latest technology and led by trained professionals who provide comprehensive wound assessment, treatment, and healing plans outside the traditional hospital environment.


Our services are ideal for individuals with chronic, non-healing wounds, residents of nursing homes or rehabilitation centers, patients with mobility limitations, or anyone who prefers to receive wound care in the comfort and privacy of their own home.


We offer a wide variety of advanced wound care services including:

We accept most major insurance plans. Please contact us to verify if we accept your specific plan.


Many insurance plans—including Medicare, Medicaid, and most private insurers—cover medically necessary wound care services provided in the home or facility. Our administrative team will work with you to verify your benefits, obtain any required authorizations, and explain your coverage and out-of-pocket costs.


While referrals are often helpful, they are not always required. You may contact us directly, and we can coordinate with your existing healthcare providers as needed.


We collaborate closely with your physician to deliver advanced wound care services that often extend beyond the scope of standard home visits. Many primary care providers and specialists do not routinely perform specialized procedures such as debridement, application of skin substitutes, or negative pressure wound therapy (i.e., wound vacs). By partnering with Elite Wound Experts, your doctor ensures you receive expert-level wound care while they continue to oversee your broader health needs.

Referral Information

 We have three easy ways to submit a referral: 

  1. Use our online referral form
  2. Download and complete the new patient forms and submit via:

1. Complete the Referral Form 

  • Use our online referral form or download a PDF copy and complete.


2. Submit Referral Information 

  • Submit the referral form along with all required documentation using one of the three options listed below.


3. Next Steps

  • Our team will verify insurance after receiving all documentation. Once verifed, the patient will be scheduled for an initial visit within 2–3 business days. 


4. Ongoing Communication

  • Weekly progress reports will be sent to the referring provider. 


Include the following information when making a referral:

  • Patient facesheet
  • Insurance information
  • List of current medications
  • List of allergies
  • Applicable labs/tests (last 3 months)
  • Wound photos (color photos preferred)
  • Wound Care notes (minimum of 1 week of wound care documentation)


Additionally, we request/PREFER the patient consent form be signed and completed


Once we receive all required documentation, our team will begin the insurance verification process. Once insurance is verified, we will reach out to the patient to schedule the initial visit (within 2-3 business days).


We understand the importance of ongoing communication - weekly progress reports will be sent to the referring provider. 


Typically 3-5 business days.  First we verify insurance and may have to gather more information for the insurance provider prior to being approved.  Once approved, we will get you on our schedule immediately and our clinical team will provide you with updates and scheduled visit times. 


Care and Treatment Process

During your initial visit, our wound care specialist will conduct a comprehensive assessment of your wound and overall health. This includes reviewing your medical history, evaluating the wound's size, depth, and condition, and discussing your treatment goals. Based on this assessment, we will develop a personalized care plan and begin appropriate treatment.


Our team consists of board-certified advanced practice providers, all experienced in advanced wound healing techniques. We collaborate closely with your primary care doctor and other members of your healthcare team to ensure seamless, coordinated care.


Absolutely! We encourage family involvement and provide education and support to caregivers so they can assist with wound management and participate fully in the healing process.


Visits typically occur once per week. If a Home Health agency is also involved in the patient’s care, we coordinate with their team to ensure coverage for the remaining six days. Our practitioners document each visit thoroughly, communicate the treatment plan, and provide regular updates to the Home Health team and any other healthcare providers involved in the patient’s care. 


We adhere strictly to all healthcare privacy laws and best practices, including HIPAA guidelines. All our staff are trained in infection control and safety protocols to protect the health of our patients and their families.
